Medical Laboratory Science program requirements

General information

  • Immunization shots and tuberculosis (TB) tests are mandatory for all students in the Medical Laboratory Science (MLSc) program. You will be denied entry into the laboratory if documentation has not been submitted.
  • Please refer to the links at the bottom of the page to print the necessary forms and obtain information for your health-care provider.
  • Information about completion of the Entry Immunization Form (EIF) will be will be provided during a tutorial session of HLSC 2110U Foundations in Clinical Exercise Biochemistry in the Winter semester of the first year in the MLSc program.
  • All students in the MLSc program are required to have a criminal reference check (with vulnerable sector screen) completed prior to the start of their practicum placement. If you have not provided appropriate documentation of a completed background check, you will not be eligible for practicum placement. Background checks are considered current if completed within one year of the placement start date.
  • Exceptions to these requirements are non-negotiable due to liability and safety regulations.

Document handling

  • Students are responsible for keeping the original documents at all times. When immunization forms are completed, students must show all documentation to Marsha Townsend (visit the Science Building, Room 3027, or email marsha.townsend@uoit.ca).

Note: Students must monitor their UOITnet email addresses throughout the entire year to facilitate the smooth handling of their documentation.
