Pre-practicum placement requirements

The following chart indicates the pre-placement requirements for all students—Bachelor of Science in Nursing (BScN) and Registered Practical Nurse (RPN)-to-BScN programs—as well as the frequency with which they are required.

Submission:

Students will be asked to present all required documentation to the Nursing office, located in the Science Building, Room 3042, for verification by the specified due date. 

An Entry Immunization Form (EIF) is required for all new students.

The EIF is a condition of acceptance into the practicum component of the program. This document outlines all immunizations required and must be completed by a health-care provider. Completion can take up to four weeks and may require more than one visit. Students should plan accordingly, keeping in mind the specified due date for verification of requirements. 

If you do not have access to a health-care professional, the Campus Health Centre (CHC) can help you complete this form. The CHC is open Monday to Friday from 9 a.m. to 4:30 p.m. Contact the CHC at 905.721.3037.

The cost of the appointment is covered by the Ontario Health Insurance Plan (OHIP); however, there is a fee for TB testing (this is not an insurable service through OHIP).

Clients without OHIP coverage will be charged a fee for these services.

If you reside in Durham Region, please print and complete the Durham Police CRC Letter and take to your local police station.

If you reside in Toronto, Ontario, the CRC application form can be picked up outside of the Nursing office in the Science Building, Room 3042 or from the Student Services Centre in the Science Building, Room 2000. It must be signed by an authorized University of Ontario Institute of Technology representative prior to submitting to the Toronto Police Service.

  • A level-C or health-care provider (HCP) cardiopulmonary resuscitation (CPR) certification/re-certification is required annually and is valid for one year based on the date indicated on the certification card.
  • Mask-fit testing is required every two years; testing for the BScN students only will be arranged in the Fall semester.

RPN-to-BScN students: Most students receive testing through their places of employment and should verify their mask fit annually with the Nursing office. If you need a test, please contact the Nursing office in the Science Building, Room 3042 for a list of companies. Students are responsible for any charges incurred.

  • The influenza vaccine is not mandatory but strongly recommended each year. Not being vaccinated may have implications for the student’s continuation in placement if an outbreak occurs. 

It is the student’s responsibility to keep original copies of all required documents. Students could be asked for proof at any time throughout their program. 

Please note: Students who have not verified their pre-placement requirements by the due date specified will be removed from the upcoming placement and will be withdrawn from their enrolled practicum course. Due to liability and safety reasons, there are no exceptions to this program policy.
