Incident reporting

An incident is an event that actually or potentially affects the safety of an individual or individuals. This does not include personal injury to the student. Reportable incidents include but are not limited to:

  • medication and treatment errors
  • client falls
  • equipment failures

In the event of an incident, the student will:

  1. Ensure the safety of the client.
  2. Report the incident to the co-assigned nurse or preceptor and the clinical faculty.
  3. Follow all agency policies.
  4. Document in the patient's health record an objective, factual account of the incident with the assistance of the co-assigned nurse/preceptor and/or clinical faculty.
  5. In consultation with your clinical instructor, co-assigned nurse or preceptor, discuss disclosure of the error.
  6. Under the direction of the co-assigned nurse/preceptor and/or clinical faculty, complete the factual account of the event on the agency incident report.
  7. Complete the UOIT/DC/GC Incident Report.
